Qualifications

• Must hold CNA certification and be able to read, write and speak English fluently.
• 1–2 years’ home health/hospice aide or nurse aide experience preferred.
• Completion of a home health/hospice aide training or in-house competency program.
• Must be able to effectively communicate and follow instructions in a team setting.
• Reliable transportation and ability to work with diverse patient needs.

Responsibilities

• Provides basic personal care and homemaking services as directed by RN.
• Assists with bathing, grooming, meals, and light housekeeping in patient homes.
• Supports mobility, positioning, range-of-motion, and safety.
• Assists with medications typically self-administered as set up by caregiver.
• Documents care and communicates changes to the team in a timely manner.
