Job summary

Omatochi is seeking a Caregiver to provide direct care in clients’ homes and community settings, following care plans to support daily living activities with safety and respect. This role requires reliable transportation within the service area and adherence to documentation standards.

You will document services using mobile systems, communicate with families and the care team, and maintain professional boundaries while delivering compassionate, client-centered care.

Qualifications

• Minimum 1 year of caregiving or related experience
• Ability to follow care plans and instructions
• Reliable transportation within service area
• Ability to pass background check and required screenings

Responsibilities

• Provide personal care and support services as outlined in client care plans.
• Assist clients with activities of daily living as assigned.
• Maintain a safe and respectful environment for clients.
• Accurately document services delivered using approved systems.
• Report incidents, concerns, or changes in client condition immediately.
• Follow documentation and reporting standards at all times.
