Responsibilities

• Provide patient-centered care tailored to each client’s needs and preferences.
• Assist with daily living activities including bathing, dressing, grooming, and mobility support.
• Prepare meals and snacks as needed.
• Provide medication reminders and assist with medication management as directed.
• Offer companionship and activities to encourage social interaction and mental stimulation.
• Perform light housekeeping, run errands, and other supportive tasks.
• Report changes or concerns to the care coordinator or supervisor.
