Job summary

ComForCare Home Care in New Jersey seeks Registered Nurses to support clients in home settings across Somerset and northern Middlesex Counties. You will lead assessments, develop care plans, and supervise CHHAs to ensure safe, high-quality care for patients, including those with dementia or Parkinson's disease.

15–20 hours weekly on weekdays with some weekend flexibility, travel within service area, mileage reimbursement, and a company-provided tablet/laptop support field work.

Qualifications

• Active RN license in good standing.
• Experience in home care, geriatrics, or chronic/neurological conditions preferred.
• Strong clinical judgment to translate assessments into care plans.
• Comfortable with laptops/tablets, EMR systems, and Google Workspace.

Responsibilities

• Conduct in-home clinical assessments to evaluate health status, safety, and well-being.
• Develop and oversee individualized care plans that promote stability and quality of life.
• Identify and mitigate risks within the home to prevent decline and hospitalizations.
• Administer medications, provide wound care and skilled nursing tasks per orders.
