Qualifications

• Registered Nurse (RN) with a bachelor’s degree in nursing (preferred).
• 1+ year of clinical experience as a registered nurse (homecare experience a plus).
• Strong verbal and written communication skills.
• Proficient with technology – EMR systems and digital tools.
• Reliable car and valid driver’s license for travel to patient homes.

Responsibilities

• Complete a two-week clinical orientation and learn our EMR and safety protocols.
• Assess patients with varying health needs and develop personalized care plans.
• Educate patients and families to improve health outcomes and self-management.
• Collaborate with a team of VNA clinicians and external providers for seamless care.
• Document changes in patient conditions and update care plans accordingly.
