ComForCare Home Care in Somerville, NJ is seeking compassionate RNs to support clients in the home setting. This role offers flexible scheduling with 15–20 hours per week, and potential to extend to full-time or two part-time positions, focusing on proactive, person-centered care.

You’ll conduct in-home assessments, develop care plans, coordinate with families, supervise CHHAs, and document care using EMR systems.

Qualifications

• Active RN license in good standing.
• Experience in home care or geriatrics preferred.
• Strong clinical judgment and care planning.
• Proficient with EMR systems and Google Workspace.
• Willingness to travel across Somerset and northern Middlesex counties.
• Reliable transportation and self-directed.

Responsibilities

• Conduct in-home health assessments evaluating status and safety.
• Develop and oversee individualized care plans.
• Identify risks to prevent decline and hospitalizations.
• Administer medications and provide wound care per orders.
• Advocate for clients and coordinate referrals.
• Support families with clinical guidance.
• Supervise and train CHHAs to ensure quality care.
• Maintain continuity of care with follow-ups and communications.
• Document care in EMR accurately and promptly.
