BrightStar Care is seeking a dedicated Home Care Field Specialist in the Duluth, MN area. The role involves providing high-quality home care services to clients with diverse needs, including assisting with daily activities and mobility.

Comfortable with Hoyer lifts and reliable transportation are required, with a 40-hour weekly availability to ensure coverage. The position offers 30 guaranteed hours per week and travel within a 35-mile radius of Duluth to service clients in Duluth and

Qualifications

• Compassionate and patient caregiver mindset.
• Strong interpersonal and communication skills.
• Experience providing hands-on care in Home Health, Assisted Living, or similar settings.

Responsibilities

• Provide companionship and conversation to enhance client lives.
• Assist with daily activities such as meals, light housekeeping, and errands.
• Provide physical assistance including bathing, dressing, grooming, transfers and ambulation.
• Accompany clients to appointments or social outings and provide transportation as needed.
• Monitor and report changes in health, behavior, or needs to RN Case Manager.
• Collaborate with Care coordination team to update the care plan.
