Qualifications

• Current certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA).

• Ability to lift 50 pounds and stand for long periods.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsibilities

• Provide high-quality, patient-centered care to clients in their homes, focusing on their physical, emotional, and social well-being
• Assist with activities of daily living, such as bathing, dressing, and grooming, as well as light housekeeping and meal preparation
• Monitor and report changes in clients' condition, ensuring prompt communication with healthcare providers and family members
• Collaborate with a multidisciplinary team to develop and implement personalized care plans
• Maintain accurate and detailed records of client care and services provided
