Job summary

Active Day is seeking a Personal Care Aide to provide, restore, maintain, and promote clients’ health through home support, medical monitoring, transportation, and assistance with daily living activities.

The role emphasizes a positive, dependable attitude, strong communication, and flexible scheduling to help keep members safe and improve health and happiness of clients and families.

Qualifications

• High school diploma or equivalent.
• For positions involving driving: valid driver’s license and reliable transportation are required.
• Background check and state regulatory requirements may apply.
• Minimum one year full-time equivalent healthcare experience preferred.
• Prior experience with seniors or adults with special needs preferred.
• Current Home Health Aide and/or CNA certification preferred.

Responsibilities

• Provides support for activities of daily living and personal care (e.g., arrival/departure, eating, ambulation, transferring, therapies, rest room assistance).
• Perform home support tasks (light cleaning, laundry, shopping, home safety).
• Prepare and serve meals and snacks; assist with feeding as needed.
• Observe for changes in condition; escalate concerns as appropriate.
• Run errands for clients (shopping, appointments).
• Ensure safety by maintaining a sanitary, clutter-free environment.
• Communicate positively with members, families, co-workers, referral agencies, and the community.
