Job summary

Baptist Trinity Hospice in Tennessee, part of the Optum family, seeks in-home care professionals to provide personal care and support to clients in their homes. The role emphasizes safety, documentation, and compassionate service in a home setting.

Ideal candidates have a driver’s license, reliable transportation, CPR if possible, and 6+ months of home care or related experience. Flexible hours and independent work are required.

Qualifications

• Current driver’s license, vehicle insurance, and reliable transportation.
• CPR certification preferred (within 90 days of hire).
• 6+ months of home care experience preferred.
• Ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders.
• Support household tasks, meal preparation, and accompany clients to appointments or errands as needed.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
