Qualifications

• Must have NYS Home Health Aide Certificate.
• Reliable transportation may be required.
• TB screening and immunization requirements per policy.

Responsibilities

• Daily personal care (bathing, grooming, oral care, dressing).
• Assist with activities of daily living per Plan of Care.
• Monitor patient condition and report changes.
• Assist with meals and basic housekeeping.
• Accompany patient to doctor visits and runs essential errands.
