Job summary

Summit Home Health Care in New York is seeking HHA/PCA certified caregivers to provide exceptional in-home care for our patients. We value trustworthy, team players who can work with Summit to deliver outstanding service.

Responsibilities include daily personal care, assisting with activities of daily living, monitoring patient conditions, helping with meals and light housekeeping, and accompanying patients to appointments and errands. NYS HHA certificate required along with other documentation.

Qualifications

• Must have New York State Home Health Aide Certificate.
• Government Issued ID, Social Security Card & Work Authorization, if needed.
• 2 Recommendation Letters.
• Recent Physical.
• TB Screening (Quantiferon preferred).
• Flu shot.
• Rubella & Rubeola titer lab work.
• Chain of Custody Drug Screen no older than 6 months.

Responsibilities

• Daily personal care (bathing, grooming, oral care, dressing, etc.).
• Assist with the daily living activities per the Plan of Care.
• Monitor patients and report on their condition.
• Help with patient meals and feeding, as well as basic housekeeping functions.
• Accompany the patient to doctor's visits, shopping, and other essential errands.
