Job summary

Summit Home Health Care is seeking compassionate Home Health Aides in New York to provide daily personal care and support to patients. Join a team that values trust, reliability, and collaboration to deliver exceptional care.

Responsibilities include assisting with activities of daily living, accompanying patients to appointments, monitoring conditions, and helping with meals. Requirements include NY State HHA certificate, government ID, SSN, two recommendations, and health screenings.

Qualifications

• Must have New York State Home Health Aide Certificate
• Government Issued ID, Social Security Card & Work Authorization, if needed
• 2 Recommendation Letters
• Recent Physical
• TB Screening (Quantiferon preferred)
• Flu shot
• Rubella & Rubeola titer lab work
• Chain of Custody Drug Screen no older than 6 months

Responsibilities

• Daily personal care (bathing, grooming, oral care, dressing, etc.)
• Assist with daily living activities per the Plan of Care
• Monitor patients and report on their condition
• Help with patient meals and feeding, as well as basic housekeeping functions
• Accompany the patient to doctor’s visits, shopping, and other essential errands
