Interim HealthCare - Dubois, PA is seeking a Home Health Aide/Certified Nursing Assistant to join our home-based care team. The role involves hands-on personal care, assisting with daily living activities, reporting changes in patient condition, and supporting therapy or nursing services in the home setting.

Responsibilities include taking vitals, aiding in nutrition and self-care, ensuring safety, assisting with mobility, and documenting care provided in a timely manner.

Qualificações

• Two positive references.
• Evidence of passing a home health aide written knowledge exam.
• Pass state required criminal and abuse background checks where required.
• Able to effectively communicate in English with all levels of the workforce, both clinical and non-clinical.
• Able to read, write, and verbally report clinical information to patients, caregivers and staff.
• Able to perform and prioritize multiple tasks.
• Proof of valid driver's license if applicable.
• Proof of valid auto liability insurance if assignment includes driving.

Responsabilidades

• Reads and records temperature, pulse, and respirations.
• Assists with activities of daily living (bathing, grooming, toileting, nutrition).
• Promotes safe environment and unobstructed pathways.
• Assists with ambulation, transfers, and range of motion exercises.
• Assists with incidental activities of daily living (shopping, meals, homemaking).
• Medication reminders and assisting with self-administration.
