Job summary

White Mountain Apache Behavioral Health Services, Inc. seeks an Integrated Health Transportation Driver to provide safe, reliable transport for clients to behavioral health appointments and services within the Integrated Health program on the Fort Apache Reservation.

The role requires vehicle safety checks, adherence to tribal and state laws, HIPAA compliance, and coordination with Integrated Health staff. Experience with wheelchair access and emergency contacts is a plus; we value punctuality

Qualifications

• High school diploma or GED required.
• Valid Arizona driver's license and clean driving record.
• Ability to assist clients with entering/exiting vehicle safely.
• Basic computer skills for transportation documentation.
• Must complete pre-employment requirements including background check.

Responsibilities

• Provide safe, reliable transportation for clients to and from appointments.
• Coordinate pickups/drop-offs with Integrated Health staff and manage schedules.
• Complete AHCCCS Daily Trip Report forms with trip and vehicle data.
• Perform routine vehicle safety checks and report maintenance issues.
• Comply with Arizona laws, tribal policies, and ABHS procedures.
• Maintain client safety, confidentiality, and professional boundaries.
• Notify staff of no-shows, delays, or safety concerns in a timely manner.
• Assist with local dialysis transport if needed and as assigned.
