POSITION: INTERGATED HEALTH TRANSPORTATION DRIVER
CLASSIFICATION CODE: NON-EXEMPT FULL TIME
DEPARTMENT: NDEE BAH BEHAVIORAL HEALTH – INTERGRATED HEALTH
REPORTS TO: ASSISTANT DIRECTOR OF INTERGRATED HEALTH
STATEMENT OF JOB:

The Integrated Health Transportation Driver is responsible for providing safe, reliable, and timely transportation services for clients receiving services through Ndee Báh Behavioral Health – Integrated Health. The position coordinates and provides transportation for clients attending scheduled behavioral health appointments and services at Ndee Báh Behavioral Health – Integrated Health, maintains required transportation and vehicle documentation, and communicates with staff regarding transportation needs and schedules.

The primary responsibility of the position is to support transportation needs for Ndee Báh Behavioral Health. As needed, the driver may assist with local transportation of Ndee Báh Dialysis (Whiteriver) patients from the dialysis center to their residence following treatment when CBO transportation is unavailable or delayed due to limited transportation. Dialysis transportation will be provided as operationally appropriate and will remain secondary to Ndee Báh Behavioral Health transportation needs.

The Integrated Health Transportation Driver must comply with all applicable safety laws, regulations, and agency requirements while operating an agency vehicle within Arizona and on the Fort Apache Indian Reservation. Agency vehicles must be used exclusively for authorized Apache Behavioral Health Services (ABHS) business. The driver must follow all ABHS policies and procedures related to vehicle use, transportation, and safety.

DUTIES AND RESPONSIBILITIES:
  • Provide safe, reliable, and timely transportation for Ndee Báh Behavioral Health – Integrated Health clients to and from approved behavioral health appointments, services, and other authorized locations.
  • Coordinate transportation needs with Integrated Health staff, including scheduled pickups, drop-offs, appointment times, cancellations, and changes to transportation schedules.
  • Complete and maintain accurate AHCCCS Daily Trip Report forms for all client transports, including required trip information, mileage, vehicle information, and other required transportation documentation.
  • Conduct routine vehicle safety checks, maintain the cleanliness and general condition of the assigned agency vehicle, and promptly report vehicle damage, maintenance concerns, safety issues, or mechanical problems.
  • Operate agency vehicles in compliance with Arizona State and White Mountain Apache Tribal traffic laws and requirements, including speed limits, hands-free requirements, traffic restrictions, checkpoints, and detours.
  • Maintain client safety during transportation by ensuring seatbelt use, driving safely, and remaining alert to traffic, road conditions, client behavior, and other potential hazards. When safety is at risk, stop in a safe location and follow ABHS procedures, including contacting a supervisor, Tribal Police, or 911 as appropriate.
  • Maintain professional and respectful interactions with clients, staff, community partners, and others while maintaining appropriate professional boundaries.
  • Communicate client no-shows, unsuccessful pickup attempts, cancellations, transportation delays, safety concerns, and other transportation-related issues to Ndee Báh Behavioral Health – Integrated Health staff in a timely manner.
  • As needed, assist with local transportation of Ndee Báh Dialysis (Whiteriver) patients home following dialysis treatment when CBO transportation is unavailable or delayed. Ndee Báh Behavioral Health – Integrated Health will remain the driver’s primary assignment.
  • Abide by all applicable policies, procedures, and program standards of Apache Behavioral Health Services (ABHS), the White Mountain Apache Tribe, Indian Health Service (IHS), Arizona Health Care Cost Containment System (AHCCCS), and other applicable regulatory authorities.
  • Maintain client information and transportation records in accordance with HIPAA, AHCCCS, ABHS, Federal, State, Tribal, and local requirements.
  • Maintain professional and ethical conduct in accordance with applicable ABHS policies, State requirements, and professional standards. Promptly report client statements, behaviors, or concerns related to suicide, homicide, self-harm, harm to others, or other immediate safety concerns to Ndee Báh Behavioral Health – Integrated Health staff in accordance with ABHS policies and procedures.
  • Maintain regular and reliable attendance to ensure scheduled transportation services are available. When requesting planned leave, coordinate transportation coverage with Ndee Báh Behavioral Health – Integrated Health team members and the supervisor in accordance with ABHS leave procedures.
  • Attend and complete all mandatory ABHS training and participate in required agency meetings and events.
    • Complete position-specific training as required, including driver and transportation safety, proper cleaning and handling of bodily fluid exposure, safe client assistance and vehicle loading, operation of wheelchair-accessible equipment and vehicle lifts, and other training related to transportation responsibilities.
    • Complete assigned Relias training as required to support job responsibilities, safety, and ongoing professional development.
  • Participate in Clinical Oversight as required.
  • Perform other related duties as assigned by the Assistant Director of Integrated Health, Integrated Health Lead Clinician, or designee.
MINIMUM QUALIFICATIONS:
  1. High school diploma or GED required.
  2. Must be at least 21 years of age.
  3. Must possess and maintain a valid Arizona Driver License, have an acceptable driving record, and meet Apache Behavioral Health Services (ABHS) requirements for operating an agency vehicle.
  4. Ability to assist clients with safely entering and exiting the agency vehicle and to lift, load, unload, and secure wheelchairs or other transportation-related equipment as needed.
  5. Ability to work professionally with individuals receiving behavioral health services while maintaining confidentiality, appropriate boundaries, and client safety.
  6. Basic computer skills, including Microsoft Office and electronic systems used for transportation documentation and communication.
  7. Must successfully complete all required pre-employment requirements, including, but not limited to, fingerprint clearance and background check.
  8. Ability to communicate in the Apache language is preferred but not required.
