Qualifications

• Active RN/LVN/LPN license in good standing.
• 2+ years of healthcare-related work experience.

Responsibilities

• Assist with assessments and admissions according to community policies.
• Draft and update Individualized Service Plans (ISP).
• Coordinate medications delivery and pharmacy communications.
• Provide training and education on medication delivery, safety, and procedures.
• Perform basic nursing functions and resident care tasks as assigned.
• Ensure accurate documentation and regulatory compliance.
• Assist with community marketing and family communication.
