Job summary

Qualifications

• Health Care Aide Certificate from a recognized program

Responsibilities

• Assist clients in activities of daily living, maintaining self-respect and independence.
• Follow individualized care plans and report changes to the team leader.
• Document care according to policy and update health records.
• Provide or assist with personal hygiene as per care plan.
• Assist with elimination, mobility and feeding as required.
• Maintain safety and infection control, including hand hygiene.
• Support admissions, transfers and discharges and organize client belongings.
• Organize and facilitate activities for individuals or groups as needed.
