Responsibilities

• Assist clients in activities of daily living, encouraging independence and dignity while following client records and care plans.
• Follow individualized care plans, provide feedback to improve care, and report changes to the team leader.
• Document care according to policy and record observations in the client health record.
• Provide or assist with personal hygiene as per care plan, including bathing, grooming, nails, skin care, and dressing.
• Assist with elimination, mobility, and safety measures per policy and procedure.
• Observe and record client measurements, vital signs, intake/output, and notify changes to the team leader.
• Maintain a clean environment, stock supplies, and ensure safe handling of equipment.
• Participate in admissions, transfers, and discharge processes and protect client privacy and confidentiality.
• Support infection control practices and hand hygiene; ensure security and proper use of equipment.
