Qualifications

• Health Care Aide Certificate from a recognized program.
• Other combinations of education and experience may be considered.

Responsibilities

• Assist clients in activities of daily living while promoting independence and dignity.
• Follow individualized care plans and report client changes to the team leader.
• Document care according to policy and in health records.
• Provide or assist with personal hygiene as per care plan.
• Assist with elimination, mobility, feeding, and safety per care plan.
• Maintain a safe environment and infection control practices.
• Order, stock, and organize unit supplies; keep environment clean.
• Assist with admissions, transfer, and discharge of clients.
