Job summary

Qualifications

• Current state LPN license required.
• 1 year of LPN experience in community-based care preferred.
• Follow state regulations and company policies.
• Must pass criminal background check.
• LPN students meeting criteria may be considered.”

Responsibilities

• Follow physician orders and provide detailed assessment data for resident care.
• Record patient medical history accurately.
• Take and record measurements (blood pressure, temperature, heart rate).
• Observe patients under treatment to identify progress and side-effects of medications.
• Document changes in patients’ condition and notify physician and supervisor.
• Administer injections, prescribed medications, enemas, clean/dress wounds and assist with basic care.
• Collect lab samples as assigned.
• Provide emotional and psychological support.
• Communicate with patients’ family or friends to provide advice and release instructions.
• Utilize various clinical skills in geriatric care including wound care, infection control, IVs, wound vacs, gastrostomy tubes, medication management.
