Job summary

Homestead is seeking an LPN to provide compassionate nursing care for our residents. You will conduct assessments, follow protocols, document changes, and support the care team with empathy and integrity.

Responsibilities include admissions, discharge planning, wound care, and leading CNAs and CMTs while maintaining facility policies. You will coordinate with physicians and guardians, ensure accurate MAR/TAR/POS, and respond promptly to emergencies.

Qualifications

• LPN licensure is required.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings.
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications.
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation.
• Provide wound care: follow protocols, irrigate, pack, and remove dressings.
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies.
• Respond effectively to emergency situations.
