Qualifications

• Bachelor’s degree in a behavioral health field or related area is required.
• Valid Texas driver’s license with acceptable driving history is required.
• Ability to pass criminal background screenings and transport clients in a personal vehicle.

Responsibilities

• Conduct needs assessments and ongoing client evaluations.
• Develop and review individualized service plans.
• Connect clients with community resources and supports.
• Coordinate care with providers, families, schools, and agencies.
• Advocate for clients to obtain needed services and benefits.
• Monitor progress and adjust services as needed.
• Assist with discharge planning and aftercare.
• Maintain clinical documentation within required timeframes.
• Participate in team meetings and case reviews.
• Ensure Medicaid compliance and serve as liaison for clients and providers.
