As a Community Case Manager, you will provide comprehensive case management services to individuals experiencing mental health and behavioral health challenges in both home and community settings.
We are looking for professionals who are:
Bachelor’s Degree from an accredited college or university in one of the following: Psychology, Social Work, Medicine, Nursing, Rehabilitation Counseling, Sociology, Human Growth and Development, Gerontology, Special Education, Educational Psychology, Early Childhood Education, Early Childhood Intervention, Child Development Therapy, Human Services, Human Development, Family Studies Or a related behavioral health field
This is a primarily field-based role requiring travel throughout the community and client homes. Candidates should be comfortable working in various community environments and supporting individuals experiencing mental health crises or significant life stressors.
