Job summary

Qualifications

• Bachelor's in Social Work required, Masters degree preferred.
• California Licensed Clinical Social Worker required.
• Three to five years hospital social work experience preferred.

Responsibilities

• Supports planning and post-discharge of patients
• Screens, assesses, and provides resources regarding social drivers of health
• Acts as expert in social drivers of health resources in the community
• Refers patients to Behavioral Health as appropriate
• Provides education and support for food access and housing
• Coordinates the patient transportation program
• Assists families in crisis situations in understanding implications of medical situation
• Conducts financial assessment and referral services
• Arranges for in-home care and equipment
