Waterview Nursing Care Center is seeking an experienced Social Worker to join our dynamic skilled nursing team. You will work directly with patients and families, remaining patient, calm, and capable of managing stress with superb interpersonal communication and attention to detail.

Responsibilities include evaluating patients, facilitating care plan meetings and discharge planning, completing MDS social work sections, coordinating with nursing and rehab, and ensuring compliance with long-term

Qualifications

• Experience as Social Worker in a skilled nursing facility is preferred.
• Must be patient, calm and able to manage stress.
• Excellent interpersonal communication skills and superb attention to detail.

Responsibilities

• Evaluate patients based on their needs, limitations and desires.
• Facilitate care plan meetings and discharge planning.
• Completing MDS social work section.
• Address patient concerns and goals while maintaining communication with patients and case managers.
• Build rapport with patients and families.
• Coordinate care with Nursing and rehab departments.
• Develop care plans for patients.
• Knowledge of long-term care regulations including documentation requirements.
• Audits psychology paperwork, hospital discharges, room changes, PASRR and advanced directives.
