Job summary

Qualifications

• High school diploma or equivalent; experience with people with disabilities preferred.
• Demonstrated written and verbal communication skills.
• Must be able to lift up to 40 pounds and have reliable transportation.
• Valid driver’s license and clean driving record.
• Pass background checks and abuse registries.

Responsibilities

• Supervise, train, and assist clients with daily living to meet PCSP goals.
• Schedule medical and mental health appointments and coordinate transportation as needed.
• Complete service documentation within 24 hours of delivery, including MAR records.
• Maintain confidentiality and report progress to supervisor regularly.
• Support safety, cleanliness, and participation in community activities.
• Follow agency policies and complete required trainings.
