Qualifications

Responsibilities

• Support individuals in community activities, social events, and meaningful activities.
• Assist with daily living skills, meals, budgeting, and transportation when needed.
• Provide personal care support and monitor health and wellbeing per plan.
• Document progress and maintain HIPAA compliant records.
