Easterseals PORT Health in Charlotte, NC is seeking a Direct Support Professional to join our Group Home team. This on-site role involves assisting residents with daily living, personal care, health-related needs, and community involvement, aligned with individualized care plans.

• High school diploma or equivalent required.
• Valid driver's license and auto insurance required.
• One year of related experience preferred.
• Must be able to lift at least 50 pounds and assist mobility.
• Ability to speak clearly, write legibly, and perform repetitive motions.

Responsabilidades

• Provide person-centered support with activities of daily living, personal care, household tasks, transportation, and community engagement to promote independence and quality of life.
• Coordinate with individuals, families, supervisors, healthcare providers, and resources to support goals and safety.
• Deliver direct support services including skill-building, behavior support, meal prep, medication assistance, activities, and individualized plans.
• Document and maintain progress notes, medication records, incident reports, and health and safety observations.
• Contribute to quality care through supervision, training, and continuous improvement within the team.
