Job summary

Outcome HC is seeking a Certified Nursing Assistant in Pennsylvania to perform nursing services under RN/LPN supervision in a facility governed by PA DOH and federal regulations.

Responsibilities include assisting residents with daily routines, following care plans, and performing basic clinical tasks while maintaining HIPAA privacy and safety standards. Prior long-term care experience is preferred; English proficiency is required.

Qualifications

• Must be Certified Nursing Assistant in Pennsylvania.
• Ability to read, write and comprehend English.
• Long-term care experience preferred.

Responsibilities

• Follow the plan of care and attend assigned meetings.
• Provide personal care (bathing, nails, grooming) and assist residents with clothing.
• Follow infection control procedures and maintain privacy (HIPAA).
• Assess comfort levels and provide restorative care as needed.
• Provide routine catheter care and care for residents with tubes or special equipment.
• Transport residents to activities and respond to call bells.
• Record water intake and provide snacks; monitor smoking periods as assigned.
• Assist with meals and fall prevention protocols.
• Collect specimens and report changes to the nurse.
• Maintain room cleanliness and accompany residents to外medical visits.
• Adhere to safety protocols and reflect needs in ADL tracker.
• Other duties as assigned.
