Qualifications

Responsibilities

• Provide personal care (grooming, bathing, dressing, oral care) of residents daily and as needed.
• Change bed linen and maintain grooming supplies in resident rooms.
• Ambulate and transfer residents using assistive devices and body mechanics.
• Record resident vital signs, weights, intake/output, and food consumption.
• Routinely turn and position residents.
• Observe residents and report changes to Charge Nurse.
• Assist in collecting data for MDS and care plans; provide input to nurses.
• Provide adequate hydration to residents at regular intervals.
• Prepare residents for meals and transport to dining room; assist feeding if needed.
• Keep accurate records of food intake and output.
• Assist with aseptic and sterile techniques to prevent infection.
• Maintain confidentiality of resident and facility records.
