Job summary

Gallatin Nursing & Rehab is seeking a dedicated Certified Nursing Assistant to provide routine daily nursing care per resident care plans. You will assist with daily activities, monitor health status, documentcare, and maintain a safe, supportive environment for residents.

Must hold a valid CNA license per state requirements; CPR is preferred. The role involves bedside care, documentation, and collaboration with nursing staff in a Kentucky facility.

Qualifications

• Must be able to read, analyze, and interpret general materials.
• Must be able to write reports and procedures clearly.
• Ability to present information and respond to questions from managers and staff.

Responsibilities

• Identify residents before administering treatments or meals using wristband or photo ID.
• Record entries accurately on charts and flow sheets.
• Report changes in residents' conditions to the Nurse Supervisor/Charge Nurse promptly.
• Maintain confidentiality of resident information and report any disclosure concerns.
• Observe residents for pressure areas and skin breakdown; report as needed.
• Assist with daily living activities and mobility as trained.
• Assist with meals, hydration, and food arrangements as necessary.
• Attend nursing reports and follow established policies and procedures.
