Job summary

Westwood Post-Acute in San Jose, CA is seeking a Certified Nursing Assistant to support residents with daily activities and nursing care under supervision. A CA CNA license and current BLS/CPR are required.

Qualifications

Responsibilities

• Identify residents by wristband or photo before administering treatments or meals.
• Record entries on flow sheets (ADLs, I&Os, vitals, meals) in a clear, descriptive manner.
• Notify Nurse Supervisor/Charge Nurse of any changes in resident condition.
• Transport residents to activity locations as needed.
• Monitor and supervise residents to ensure safety at all times.
• Assist in passing meal trays and ensure correct tray for each resident.
• Support hydration programs and assist patients during meals.
