Job summary

Westwood Post-Acute in San Jose, CA is seeking a compassionate Caregiver to provide hands-on daily living assistance, mobility support, and health monitoring under licensed supervision. You will help residents maintain dignity, safety, and well-being while documenting care and adhering to facility policies.

The role requires strong communication, multitasking, and a basic understanding of healthcare procedures.

Qualifications

• Previous caregiving experience preferred.
• Strong communication and interpersonal skills.
• Ability to handle multiple tasks and prioritize effectively.
• Excellent observation and problem-solving skills.
• Basic understanding of healthcare procedures and protocols.
• Must be able to pass a background check and drug screening.

Responsibilities

• Provide hands-on care and support to residents, including daily living activities.
• Assist with mobility, transfers, personal hygiene, and health monitoring.
• Monitor residents' health under licensed staff, including vital signs and medication assistance as permitted.
• Document care provided and report changes or concerns to supervisor.
• Participate in care discussions with the healthcare team to support quality resident care.
• Safeguard residents’ belongings and maintain a safe environment; ensure call systems are accessible.
• Follow infection control, safety procedures, PPE use, and complete required training.
