Job summary

Neighborhood Healthcare PACE in Murrieta, CA is seeking a CNA/PCA to support personal care, companionship, and escort services for participants attending medical appointments. Qualifications

Responsibilities

• Performs and records routine participant care tasks including vital signs and ADL support.
• Assists with admissions, transfers, and discharges by safe escorting and positioning.
• Provides personal care: toileting, bathing, eating, grooming, dressing.
• Assists with meals, snack service, and record-keeping.
• Escorts participants to medical appointments and pharmacy drop-offs/pick-ups.
