Qualifications

• Must hold CNA or PCA credentials.
• Experience providing personal care and activities of daily living.
• Reliable attendance and clear communication.
• Able to follow a care plan and report changes.
• Experience with mobility aids is a plus.

Responsibilities

• Personal care: bathing, grooming, dressing, hygiene, toileting, and incontinence care.
• Ambulation, transfers, gait belt and Hoyer lift assistance when appropriate.
• Meal preparation, companionship, social engagement, and light housekeeping.
• Transportation and accompaniment to errands and medical appointments.
• Medication reminders and prescribed ROM assistance.
• Blood glucose monitoring assistance, Foley care, hospice support when trained.
• Follow the care plan and report changes or safety issues.
