Job summary

Trinity Health Senior Communities in Wisconsin is seeking a caring Nursing Assistant to support residents with daily living activities and to participate in admitted transfers and discharges.

You will follow care plans, assist with grooming, mobility, and meals, document care activities, and uphold safety and PPE standards. Strong communication and customer service are essential, with preferred experience in long-term care.

Qualifications

• High School Diploma or equivalent.
• Active Nursing Assistant certification in Wisconsin.
• 1 year healthcare experience preferred.
• Long-term care or geriatric experience preferred.
• Strong verbal and written communication and customer service.

Responsibilities

• Assist residents with daily living activities per care plan.
• Support admission, transfer and discharge processes.
• Assist with dental care, feeding, bathing, dressing and grooming.
• Assist with lifting, turning, moving and transporting residents using equipment.
• Perform restorative and rehabilitation exercises as instructed.
• Record data and respond to resident calls promptly.
• Follow safety precautions and wear PPE as required.
• Wear safety equipment during transfers per plan of care.
