Job summary

Trinity Health in Wisconsin is seeking a Nursing Assistant to support admission, transfer, and discharge processes and assist Elders with daily living activities, in a long-term care setting.

You will follow policies, perform ROM exercises, help with mobility, and ensure safety and high-quality resident care. Candidates should have a High School diploma or equivalent and current nurse aide certification, with strong customer service and communication skills.

Qualifications

• High School Diploma or equivalent required.
• Active Nursing Assistant certification in the state.
• Minimum of one year healthcare experience preferred.
• Experience in long-term care or geriatric care preferred.
• Superior customer service and professionalism.
• Outstanding verbal and written communication skills.

Responsibilities

• Perform tasks per policies and supervisor instructions.
• Support admission, transfer, and discharge processes.
• Assist elders with daily living activities (dental care, feeding, bathing, dressing).
• Assist with lifting/moving residents and positioning.
• Perform restorative procedures and ROM exercises; record data.
• Follow safety precautions and use PPE as required.
• Wear safety equipment during transfers per care plan.
