Job summary

SSM Health is seeking a part-time Certified Nursing Assistant to assist in a long-term care setting at The Sarah Community. The role involves direct patient care under the supervision of a registered nurse and requires completing an approved nurse aide training program with certification in the respective state.

The position supports daily patient care activities, data collection for assessments, discharge preparation, and collaboration with the nursing team to ensure safe and quality care.

Qualifications

• Completion of an approved nurse aide training program.
• No experience required.
• Certification as CNA in the applicable state (Illinois/Missouri/Oklahoma/Wisconsin) as required by location.

Responsibilities

• Receives patient care assignments from the RN in charge and initiates delegated tasks within demonstrated competence.
• Collects and documents data for admissions and reassessments for the patient population.
• Contributes to safety and quality care by charting and communicating with team members.
• Informs licensed staff of changes in patient condition or needs and patient education needs.
• Assists with activities of daily living and basic patient care tasks as directed.
• Prepares patients for discharge and accompanies them upon discharge as indicated.
