Job summary

Horizon Home Health & Hospice Twin Falls & Burley, Idaho is seeking compassionate Certified Nursing Assistants to join our home-based care team. You will assist patients with daily living activities, provide companionship, and support dignity and quality of life in their homes under supervision of licensed clinicians.

Qualifications

• Current Idaho CNA certification is required.
• CPR certification required or able to obtain on hire.
• Valid driver's license and reliable transportation.
• Must pass background screening and drug testing.
• Strong interpersonal and communication skills.
• Ability to work independently with a prescribed care plan.
• Prior home health, hospice, long-term care, or healthcare experience preferred.
• Experience with geriatric and medically complex patients preferred.

Responsibilities

• Assist patients with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, ambulation and transfers.
• Provide companionship and emotional support.
• Assist with light housekeeping related to patient care.
• Monitor and report changes in patient condition to the nursing team.
• Document services according to agency policies.
• Follow infection control, safety, and confidentiality protocols.
• Support patients and families with professionalism, respect, and compassion.
• Travel to patient homes within the assigned service area.
