Job summary

Heritage-Woods-of-Centralia in Centralia, IL is seeking a full-time Certified Nursing Assistant to provide compassionate, resident-centered care aligned with care plans and dignity.

You will assist with Activities of Daily Living (ADLs), monitor health changes with the clinical team, and engage with residents and families to create a positive experience while upholding confidentiality and privacy standards. Competitive benefits are offered.

Qualifications

• High School graduate or GED equivalent required.
• Current certification from an accredited Certified Nursing Assistant (CNA) program.
• Exceptional communication and interpersonal skills.
• Must demonstrate the ability to serve with love, compassion, and dignity.

Responsibilities

• Deliver exceptional and individualized care as determined by each resident's care plan
• Provide assistance with Activities of Daily Living (ADLs) while promoting independence and maintaining dignity
• Report changes in resident health status to the clinical team
• Connect with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience
• Engage in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experiences
• Comply with all rules and regulations regarding confidentiality and privacy of resident information
• Perform any other duties as assigned
