Qualifications

Responsibilities

• Assist patients with activities of daily living, including bathing, grooming, oral care, and feeding.
• Provide mobility support with safe transfers and ambulation.
• Record and monitor vital signs per hospital protocol.
• Communicate changes in patient condition to the RN promptly.
• Maintain a safe, clean environment and proper infection control precautions.
• Document care in the EMR in a timely manner.
• Collaborate with nurses and the interdisciplinary team to support patient care.
• Support patient and family education as directed by the nurse.
• Ensure supplies are stocked and equipment is available.
