Job summary

Trinidad Rehabilitation and Healthcare Center LLC in Trinidad, CO is seeking compassionate CNAs to join our resident-centered team and make a real difference in daily care, dignity, and comfort for each resident.

You will provide dignity-based assistance, monitor vital signs, report changes to the nursing staff, assist with activities of daily living, and help ensure residents’ safety and well‑being in a skilled nursing setting.

Qualifications

• Current CO CNA certification or certified in a compact state
• Graduate of an accredited CNA program
• A caring, reliable team player
• Passionate about providing personalized care

Responsibilities

• Provide compassionate care with dignity and respect for each resident
• Monitor vital signs and report changes to the nursing and medical team
• Routinely make rounds, providing responsive care and services to residents
• Maintain residents’ comfort, safety, and well-being in accordance with long-term care standards
• Perform other duties as assigned by management in a patient-centered care environment
