Responsibilities

Provides supportive services to nurse(s) and other staff as needed and performs duties as assigned.
Assists resident with activities of daily living per care plans and policies.
Assists with lifting, turning, moving, positioning, and transporting.
Coordinates dining room services during meals and documents intake.
Records resident weight and reports changes to nurse/supervisor.
Maintains skin condition monitoring and reports concerns.
Completes daily Point of Care documentation.
Communicates factors affecting ability to perform duties to supervisor.
