Job summary

Concord Post Acute is hiring a Certified Nursing Assistant (CNA) for full-time PM shift. You will provide resident-centered care, assist with ADLs, monitor vital signs, and report changes to licensed staff in a fast-paced skilled nursing setting.

Requirements include active California CNA certification, strong communication, teamwork, and a compassionate, dependable approach. Prior post-acute experience is preferred but not required; extensive training and mentorship are provided.

Qualifications

• Active California CNA certification is required.
• Strong communication and interpersonal skills are essential.
• Compassionate, dependable, resident-centered approach to care.
• Ability to work effectively in a collaborative healthcare environment.
• Strong organizational skills and attention to detail.
• Post-acute or skilled nursing experience is preferred but not required.

Responsibilities

• Provide high-quality care per resident care plans.
• Assist with ADLs: bathing, dressing, grooming, toileting, eating, mobility.
• Obtain and document vital signs and report changes to licensed staff.
• Respond promptly to call lights and deliver attentive care.
• Maintain a clean, safe environment for residents.
• Build positive relationships with residents, families, and team members.
