Job summary

Heirloom Assisted Living is seeking a compassionate Certified Nursing Assistant to support residents according to care plans, performing daily living activities and mobility assistance. Adhere to policies, ensure dignity and safety in care delivery.

Responsibilities include documenting intake and weight in the EMR, coordinating meals, and reporting changes to nursing staff. Must maintain infection control practices and collaborate with the care team.

Qualifications

• Certified Nursing Assistant in good standing with the state (preferred, not required).
• High school diploma or equivalent; CPR/BLS and Food Handler Permit required; no disciplinary action on certification.
• At least one year of experience in long-term care (preferred, not required).

Responsibilities

• Assist residents with activities of daily living per care plans.
• Assist with lifting, positioning, mobility, and transfers.
• Coordinate dining room services, meal delivery, and feeding assistance.
• Document meal intake and weight changes per policy.
• Report changes in resident condition to nurse and supervisor.
• Maintain infection control and safety procedures at all times.
