Responsibilities

• Assist with data collection (vital signs, height/weight, glucose) and report changes to RN.
• Assist in planning and implementing patient care per RN directions; report unusual findings.
• Promote infection control and safety; monitor for restraints per policy and medical orders.
• Document findings, interventions, and patient outcomes with signatures and timestamps.
• Assist with activities of daily living and support discharge planning.
