Qualifications

• CNA in good standing with the certification board or graduate of a state-approved CNA program and passed competency exam within 120 days
• Current CPR/BLS certification preferred
• At least one year of long-term care experience preferred
• Ability to communicate effectively in an interdisciplinary care setting

Responsibilities

• Provides supportive services to nurses and other staff as needed and performs duties as assigned
• Assists residents with activities of daily living per care plans and policies
• Assists with lifting, positioning, and transporting residents
• Coordinates dining room services during meals and documents intake
• Delivers nutritional supplements and monitors intake
• Monitors weight and reports changes to nurse
• Completes flow sheets and communicates progress to supervisor
• Keeps supervisor informed of factors affecting performance
• Reports changes in resident condition to nurse and supervisor
