Qualifications

• Certified Nursing Assistant in good standing or graduate of state-approved CNA program and passed competency exam within 120 days of employment.
• High School/GED preferred.
• Current CPR/BLS certification preferred.
• At least 1 year of CNA experience in long-term care preferred.
• Ability to communicate effectively in writing and verbally in an interdisciplinary care setting.

Responsibilities

• Provide supportive services to nursing staff and perform duties as assigned.
• Assist residents with activities of daily living according to care plans and policies.
• Assist with transferring and positioning residents and transportation as needed.
• Coordinate dining room services during meals and document intake.
• Deliver supplements and ensure adequate intake with proper documentation.
• Record resident weights and report changes to nursing staff.
• Monitor skin condition and report concerns to nurse or supervisor.
• Assist with toileting programs and complete flow sheets daily.
• Notify nurse/supervisor of factors affecting performance or resident condition.
