Job summary

Encore Communities is seeking a Certified Nursing Assistant to provide personal care to residents in a nurturing environment under the supervision of a registered nurse. You will assist with admissions, transfers, and discharges and ensure care plans are followed.

Requirements include a valid CNA license, current CPR and fingerprint clearance, and about six months of CNA/LNA experience. LTC/SNF experience is preferred. Competitive benefits and growth opportunities are offered.

Qualifications

• CNA license certification prior to hiring.
• Must have a current CPR and fingerprint clearance card.
• Minimum of six (6) months CNA/LNA experience preferred.
• Recent Long-Term Care/ Skilled Nursing Facility or Transitional Rehab experience

Responsibilities

• Assist guests/patients during admissions, transfers, and discharges. Check each guest/patients routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes and the care plan.
• Review care plans daily to determine if changes in the guests/patient's daily care routine have been made on the care plan.
